This article exposes the experience of an approach to child psychosis based on a strategy that articulates clinical work in the field of the outpatient clinic of a public hospital with a group–play device with outdoor ball. This articulation considers the game as an identification proposal. The invention of this group clinical device and the interventions carried out in this work are part of a process of deinstitutionalization of logics, of conceptions and practices that objectified the children through a historical dissociation between childhood and play. This dissociation reveals where the hospital institution works as a total institution.
